How can I become a patient?

They will walk you through the process of how to schedule an appointment, who you should see, and what forms need to be completed and brought with you. Please note that we cannot accommodate walk-in visitors.

Do I need a referral to see a MBSP physician specialist?

It varies from insurance company to insurance company. Call MBSP at (410) 266-2720 to find out. Please have the referring professional complete the MBSP Referral Form.

Do you accept new patients without insurance?

Please call (410) 266-2720 so we can discuss your situation directly. If you are self-paying, we will work with you on a payment plan.

What geographic area does MBSP serve?

MBSP accepts patients from Maryland and any other state, as long as the patient’s insurance company accepts MBSP as a provider. Please check with your insurance company prior to scheduling your visit.

MBSP has an office in Annapolis, Maryland. Check for specific days and times we see patients.

What forms do I need to fill out?

There are several registration forms and medical paperwork we ask that you complete prior to arriving for your appointment. These forms can vary from insurance company to insurance company. Call MBSP at (410) 266-2720 to find out which ones you need. You can email the completed forms to us at info@mbsp.com, or fax them to (410) 224-0209. If you can’t fill them out beforehand, plan on arriving fifteen minutes earlier to complete the forms.
